Note:  Due to COVID-19, client forms can be completed and returned electronically via email. The forms are fillable on your computer and can be digitally signed as the final step before saving and emailing the document to our office.

If you don’t have a digital signature on your computer, you will be prompted to get a digital signature (most common case is to download a New PKCS#12 digital ID file to your computer). Please download the digit signature file and complete the details. Once done, the signature can be added to the form.

As an alternative, the forms can be printed, filled out, and an photo of the forms emailed to our office.
